These templates were created by the NBDPN ELSI committee. They serve merely as “models,” and states will need to modify them to adhere to their own laws, agencies, etc. We strongly recommend each state have a legal review done in-house with its own state agency prior to utilizing a template. Each of these templates presumes a state has a law or administrative rule for reporting of birth defects and/or ascertainment of birth defects data.
